About The Position

Kitsap Mental Health Services (KMHS) is seeking a YAT (Young Adults in Transition) Team Care Coordinator to provide recovery‑focused services for clients as part of a multidisciplinary treatment team. In this role, you will plan and deliver individualized care, support families, and respond to crisis needs while maintaining strong collaboration across systems of care. You will help clients build skills, access resources, and progress toward treatment goals in community‑based settings. Requirements

  • EDUCATION: Bachelor’s degree in psychology, social services, or behavioral health field.
  • EXPERIENCE/SKILLS: Foundational (minimum one year of relevant experience).
  • This position requires driving clients in a personal vehicle on behalf of the agency. Therefore, the incumbent must meet all agency driving requirements, including being at least 21 years old, having been licensed for a minimum of three years, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver’s license in the state of Washington, and having reliable, insured transportation.
  • CREDENTIALS: Agency Affiliated Counselor Registration.

  • EXPERIENCE/SKILLS: Experienced (minimum 2 years of job-related experience) Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
  • The capacity to work well with children and families.

Responsibilities

  • Provide care coordination services for an assigned caseload, including individual, group, and family counseling, crisis intervention, and case management.
  • Assume primary responsibility for developing, implementing, evaluating, and revising individualized treatment goals in collaboration with the treatment team.
  • Educate and support families, advocate for client preferences, and consult with community agencies such as DSHS, courts, and housing providers to support continuity of care.
  • Assist clients with accessing and effectively using primary care and dental services; support development of health self‑management skills and coordination of psychiatric and medical care.
  • Provide crisis intervention counseling and coordinate with Designated Crisis Responders and crisis services when indicated.
  • Document client progress and maintain accurate records in accordance with agency policies and procedures.
  • Participate in daily organizational meetings and ongoing treatment planning reviews.
  • Provide education related to substance use, mental and physical health, and daily functioning; coordinate services with internal and external substance use treatment providers.
  • Support client autonomy by assisting with resources, education, and basic needs.
  • Assist clients in developing community‑based independent living skills, including budgeting, shopping, hygiene, cooking, cleaning, transportation access, and socialization.
  • Monitor assigned clients for LRA compliance; facilitate extensions with the CRT and court‑involved partners and assist with revocation decisions when appropriate.
