SUMMARY: Under the direction of the Clinical Program Manager, the Program Coordinator is responsible for managing the mental health referral/intake processes; for supervision and oversight of the rehabilitation, Child and Family Team (CFT) facilitation, and case management services provided to clients; and ensuring that clients are appropriately and successfully discharged and/or linked to intra-agency or community resources. The Program Coordinator promotes quality client care through a comprehensive phone screening and crisis assessment, training and developing staff, and participating in quality improvement activities. JOB DUTIES: Responds to all referrals for mental health services through the navigation of County tracking systems Conducts walk-in and telephone screenings/interviews consistent with state, county, and agency protocols, to assess for crisis, to ensure timely access to care, and to provide linkage to emergency services if needed Identifies service needs, determines eligibility, and collaborates with Clinical Program Manager(s) to facilitate appropriate case assignments Maintains tracking system for all contacts and requests for services, for both internal and external reporting purposes, via the electronic health records system Ensures all identified services are provided, and coordinates client needs with other staff members; promotes compliance with service quality standards Trains direct reports and other program staff as needed to facilitate appropriate linkage and intensive care coordination services; provides oversight of ongoing linkage to ensure clients are connected to needed services throughout treatment and at termination/transition; provides guidance alongside staff for complex case management needs Works with Clinical Program Manager(s) to facilitate service exits and/or intra-agency transitions along the continuum of care Identifies training needs of staff and works with leadership team to develop and implement orientation and training programs Reviews practices policies, and procedures of to ensure compliance with State, County, and Agency expectations, licensing requirements, and professional standards; makes corrective action as necessary Maintains confidentiality regarding children, families, and operations of the Agency Incorporates Agency philosophy and mission in all aspects of job description Performs other reasonable related duties as assigned Supervisory Responsibilities Mentors, trains, and manages a team of Child & Family Specialists and Parent Partners; develops, implements, and supervises workflows in accordance with case management needs of clients and their families Ensures proper staffing levels to ensure quality case management and advocacy service and intervenes as necessary due to lack of staff; monitors/controls hours worked; provides direct case management and supervision of clients if needed; models same when needed to demonstrate techniques to staff Delegates and supervises accurate, complete documentation of children's activities and service and ensures all reports are prepared and submitted as scheduled and in compliance with funding requirements Assists with the recruitment, interviewing, hiring, and training staff; manage performance; conducts performance appraisals and recommends salary increases; monitors and authorizes absences; takes disciplinary measures as needed Develops a high-performing department culture and staff; reviews documentation to monitor the frequency and quality of service delivery, and to ensure compliance with all state, county, and agency guidelines; provides timely feedback
