About The Position

The Wraparound Facilitator works in partnership with youth and their families, and referral sources to facilitate high fidelity wraparound services, with the goals of the youth maintaining safety in the home, school, and community. The Wraparound Facilitator utilizes a strengths-based, youth and family driven treatment approach to facilitate collaboration between all team members to ensure the youth/family's success. The Wraparound Facilitator is responsible for engaging the youth and family to provide care coordination, such as developing the family story, identifying underlying needs, building effective teams, facilitating monthly team meetings, and collaborating with team members weekly to monitor the youth and family's progress.

Requirements

  • Bachelor's degree in social work, psychology, counseling, or related field
  • A minimum of two (2) years of one or a combination of the following experiences: clinical, case management, skills building, child welfare, juvenile justice, education in a K-12 setting.
  • Ability to engage and establish positive rapport with a diverse group of individuals experiencing behavioral health symptoms.
  • Ability to work cooperatively and productively with multi-disciplinary teams and staff of partner agencies and community organizations.
  • Able to develop and maintain effective relationships with all stakeholders including the youth, family, and other service providers.
  • Demonstrates effective organizational skills.
  • Demonstrates “outside of the box” thinking skills.
  • Demonstrates strong interpersonal skills.
  • Demonstrates strong verbal and written communication skills.
  • Demonstrates sensitivity to cultural diversity.

Responsibilities

  • Incorporates the six principles of trauma informed care throughout all aspects of their work.
  • Works collaboratively with community partners to develop responsive and flexible resources that support the needs of families and youth.
  • Lives out the Wraparound Philosophy in their daily tasks, such as developing a family story, care plan, crisis/safety plan, providing monthly team meetings, crisis support, etc.
  • Assists with developing a team that supports the family through identifying and helping the family with brain-storming and problem-solving ideas to develop a functional team.
  • Meets the quality improvement measures of the program, such as but not limited to meeting the family or caregiver within three (3) calendar days of receipt of referral.
  • Partners with up to twelve families at a time.
  • Assists the youth and family accessing strength-based mental health, social services, educational services and other support and resources required to assist the family to attain its vision. They help the family identify natural supports that exist for the family, and work with families to maintain youth in the least restrictive setting possible.
  • Participates in all required training and meetings as appropriate and assigned.
  • Complies with documentation requirements according to Wraparound Philosophy and agency requirements.
  • Completes administrative tasks, such as attending training, meetings, serving on committees as assigned, engaging in DMHA site coaching as identified and according to policy.
  • Finds value in ensuring program fidelity by supporting the program's implementation and evaluation strategy.
