About The Position

POSITION OVERVIEW: Reports to the Director of Nursing and is responsible for providing day-to-day wound care and related documentation within the facility. All duties are performed in accordance with federal, state, and local regulations, facility policies, and professional standards of care. ESSENTIAL FUNCTIONS: Implements, coordinates, and monitors compliance with resident care plans involving infection control and wound care. Serves as a treatment nurse and collaborates with the wound team to assess all admissions, re-admissions, and new wounds. Ensures all admissions and re-admissions comply with federal, state, local, and facility guidelines; reviews and completes the Admissions Checklist. Evaluates and manages wounds caused by pressure, friction, disease, accidents, or surgical incisions. Assesses and monitors wounds at least weekly during healing phases; adjusts treatments as necessary and documents weekly assessments. Collaborates with the interdisciplinary team to develop appropriate treatment plans. Implements ongoing preventive measures for pressure ulcers, including staff education and weekly wound team rounds. Reviews the unit 24-hour report daily and supervises/coordinates nursing services and care plan interventions as they relate to wound care. Maintains regular and open communication with residents’ families, physicians, and wound clinics when indicated. Supports a positive and professional image through leadership activities that align with the organization’s mission and foster teamwork. Participates in developing a cohesive nursing team by motivating, guiding, and supporting staff in daily operations. Ensures all personnel involved in resident care are aware of care plans and follow them in daily practice. Ensures adherence to OSHA and safety standards in accordance with federal and state regulations, maintaining a clean and safe work environment. Complies with, and ensures staff comply with, all Infection Control, Universal Precautions, and OSHA standards. Adheres to HIPAA confidentiality standards regarding resident and client information. Attends mandatory in-services, facility, and departmental meetings as required. ADDITIONAL FUNCTIONS: Attend and actively participate in charge nurse meetings. Participate in continuing education programs to maintain professional competence and compliance with regulations and facility policies. Attend and actively participate in meetings, committees, or workgroups as directed. Other duties as assigned. KNOWLEDGE, SKILLS, AND ABILITIES: Excellent verbal and written communication skills; able to interact effectively with staff, administration, residents, families, and external contacts. Ability to remain calm and professional in complex or challenging situations. Strong organizational and time-management skills, with the ability to prioritize tasks, make decisions, delegate responsibilities, and manage multiple objectives effectively. Demonstrates patience, empathy, and a positive attitude toward the elderly population, with a commitment to serving the community.

Requirements

  • Current, unrestricted licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the Commonwealth of Virginia Department of Health Professions.
  • Current CPR certification (must be obtained within 30 days of hire if not already certified).
  • Three to five (3–5) years of nursing experience, preferably in a long-term care or senior living setting.
  • Excellent verbal and written communication skills; able to interact effectively with staff, administration, residents, families, and external contacts.
  • Ability to remain calm and professional in complex or challenging situations.
  • Strong organizational and time-management skills, with the ability to prioritize tasks, make decisions, delegate responsibilities, and manage multiple objectives effectively.
  • Demonstrates patience, empathy, and a positive attitude toward the elderly population, with a commitment to serving the community.
  • Ability to stand, walk, sit, stoop, bend, and reach for extended periods of time.
  • Ability to lift up to 20 pounds frequently and up to 35 pounds occasionally in the performance of duties.
  • Manual dexterity that allows grasping, carrying and holding equipment and supplies or for resident care as needed to perform tasks.
  • Ability to see, hear, and verbally communicate in the performance of duties.
  • Requires a satisfactory general background check, including state and local criminal history checks, as well as a satisfactory drug screening.

Nice To Haves

  • Six (6) months of previous supervisory experience preferred.
