About The Position
Lutheran Home is hiring a Wound Care Nurse (RN/LPN) to provide specialized wound and skin care within our skilled nursing community. Requirements
- Active RN or LPN license required
- Wound care certification required
- Minimum of 2 years of wound care experience, preferably in long-term care
- Strong clinical assessment and documentation skills
- Ability to follow written and verbal clinical instructions and procedures
Nice To Haves
- Experience in skilled nursing or senior living settings
- Prior experience working with interdisciplinary care teams
Responsibilities
- Provide specialized wound and skin care in accordance with physician orders and best practices
- Assess resident treatment needs and implement appropriate wound care interventions
- Monitor healing progress and adjust care plans as needed
- Educate and support nursing staff and CNAs on wound care practices
- Ensure compliance with federal, state, and local regulations and maintain survey readiness
- Collaborate with the clinical team and MDS team to support documentation and care planning
