The WIC Educator screens and enrolls eligible participants onto the WIC program and provides nutrition education, manages the participants WIC benefit issuance and offers referrals as needed. Position Summary: With a Quality, Customer First, and Compassionate approach, The WIC Educator will: Has knowledge of WIC program requirements and can explain the WIC Program to prospective participants. Establishes and maintains a caseload of participants. Performs health screenings: gathers accurate heights, weights, and hemoglobin of applicants. Collects comprehensive diet and nutrition-related health information. Evaluates information to determine nutritional risk and eligibility. Thoroughly documents client information necessary for enrollment on the WIC Program. Inputs medical, anthropometric, biochemical, and socio-economic information into the computer system. Uses multiple computer systems to collect and input data necessary for enrollment. Including but not limited to the following programs; Compass, Medicaid Portal, Peak PRO, CIIS and Electronic Medical Records. Completes a care plan for every client seen, within the documentation requirements established. Maintains a knowledge of community resources available to clients, and assesses need for and refers participants to the appropriate community resources such as Food Stamps, TANF, substance abuse programs, and nutritionists. Appropriately documents these referrals in computer system. Uses client centered counseling to educate participants. Provides individual nutrition education and implements goal setting techniques and follows-up with goals at subsequent visits. Coordinates client care with WIC Dietitians. Identifies high risk clients according to definitions and refers high risk to dietitians appropriate. Provides participant education on the rules and regulations of the WIC Program. Tailors the WIC food package to meet the medical and socio-economic needs of clients. Manages multiple demands at the same time, scheduled appointments, phones calls, walk-in clients all while managing computer systems and providing high quality customer service. Monitors the wait time and ensures participants are seen in a timely manner. Coordinates participant flow within department, and ensures participant needs are being properly addressed through scheduling. Manages phone calls from participants and agency partners, returning all calls in a timely manner. Manges referrals to the WIC program using PEAK PRO, contacting, and enrolling clients within the appropriate timeframe. Completes training to be a Certified WIC Authority in the established timeframe. Participates in ongoing WIC training. Other duties as assigned by WIC Director or WIC Supervisor.
