About The Position

Exciting opportunity to join a fast-paced medical team within a growing federally qualified community health center. HealthFirst Family Care Center, Inc . “The Heart of Our Community” provides comprehensive services including adult/pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult/pediatric dental care. WIC Community Coordinator Summary: Under the direction of the WIC Director, the WIC Community Coordinator is responsible for developing and maintaining community liaison with health and human services providers and advocacy organizations and communities served by the local WIC program; building an active, coordinated referral network and collaborative, working relationships between community agencies, groups and WIC to promote the utilization of WIC services; and assisting with retention activities to strengthen the communication with WIC participants about their eligibility with particular focus on current enrollment status and recertification. Schedule: Mon – Fri 8:30 am – 5:00 pm Must be available on some nights and weekends for community events. Pay Range: $21.00 – $22.62 per hour

Requirements

  • High School or associate’s degree graduate. At minimum a High School diploma, knowledge of community and neighborhoods served by the local WIC program and fluency in English
  • Two years of experience in community work, outreach, or health and human services preferred
  • Bilingual in Portuguese/CV Creole or Spanish preferred (specific bilingual capacity needed as appropriate to the community served by the local WIC program)
  • High level of customer service and a positive approach to work with patients.
  • Projects positive attitude about the department and the health center, and offers services to ensure positive experience
  • Ability to maintain an organized work area and work collaboratively as a member of a team
  • Must possess excellent oral and written communication skills and time management skills to effectively communicate with patients
  • Ability to handle confidential information in a professional manner
