About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency in Arizona, offering various programs including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services for individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is a non-profit provider of substance use and behavioral health programs, utilizing cutting-edge, nationally recognized, evidence-based models for prevention, education, and treatment. The Wellness Navigator's primary role is to remove personal and environmental obstacles to healthcare access. This position requires close collaboration with the primary medical provider, the clinical team, and staff from other agencies involved in patient care. The Wellness Navigator is responsible for proactively managing patient care by providing case management, education, care coordination, systems navigation, advocacy, and support, primarily delivered in-home and/or in the community. The role also involves engaging, educating, and supporting individuals, their family members, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up services.

Requirements

  • Highschool diploma or GED required
  • Current AZ Driver’s License
  • Clean 39-month MVR
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures required
  • Arizona Fingerprint Clearance card required

Nice To Haves

  • Associates degree (or higher) in a field related to behavioral health preferred
  • Education and/or experience in the field of health and nutrition preferred
  • Equivalent combination of education and experience

Responsibilities
