About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models. The Wellness Navigator will help to remove personal and environmental obstacles to health care access. The Wellness Navigator is responsible for working closely with the primary medical provider, the clinical team, and staff of other agencies involved in the patients care to proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy and support primarily in-home and/or in the community. The Wellness Navigator will engage, educate and offer support to individuals, their family patients, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up.

Requirements

  • Highschool diploma or GED required
  • Current AZ Driver’s License
  • Clean 39-month MVR
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures required
  • Arizona Fingerprint Clearance card required

Nice To Haves

  • Associates degree (or higher) in a field related to behavioral health preferred
  • Education and/or experience in the field of health and nutrition preferred
  • Equivalent combination of education and experience

Responsibilities

  • Proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy and support primarily in-home and/or in the community

  • Connect individuals, their family patients, and caregivers to prevention, diagnosis, timely treatment, and follow-up
