About The Position

Wellness Director Job Description 1. Direct and supervise all resident care and initiate corrective action as necessary. 2. Make daily resident and environmental rounds and initiate any corrective action as necessary. 3. Give daily report to personal care attendants. 4. Follow established standards of nursing practices and make recommendations for policy changes/additions to supervisor and administrator as needed. 5. Identify safety hazards and initiate corrective action immediately. 6. Assure that resident rights are being upheld. 7. Participate in facility education programs. 8. Report any staff training needs to Supervisor. 9. Identify resident problems and emergency situations and initiate immediate “life saving” measures as needed. 10. Attend staff meetings, classes and committee meetings as assigned. 11. Assume responsibility for compliance with state regulations. 12. Counsel staff and recommend disciplinary action to supervisor as needed. 13. Complete reports as directed. 14. Administer and document medications and treatments per the physician’s order and accurately record all care provided. 15. Perform and document comprehensive assessment of each resident. 16. Develop an accurate care plan based on each resident’s assessment. 17. Participate in the completion of employee performance evaluations. 18. Observe, evaluate and document all changes in residents’ conditions and report changes in residents’ conditions to physician and responsible party and take follow-up action as necessary. 19. Follow and complete facility admission procedures for new residents. 20. Perform basic nursing skills knowledgeably and competently. 21. Follow facility discharge/transfer procedures and coordinate discharge with resident, responsible party, other departments, physician and ancillary service providers. 22. Provide direct resident care and give instruction to personal care attendants related to resident care. 23. Supervise response to residents’ call for assistance and initiate corrective action as needed. 24. Listen to resident, family or physician concerns and initiate corrective action and/or report to Supervisor. 25. Count narcotics and document according to policy. 26. Complete standardized screens on residents. 27. Assist physician as indicated. 28. Prepare personal care attendants’ assignments and explain all pertinent resident care. 29. Order medications as necessary. 30. Notify physician, when necessary, of changes in residents’ conditions, weight, medication errors, medication not available, refusal of medications, abnormal lab and x-ray results and any other unusual observations. 31. Document unusual resident occurrences as they occur 32. Observe and follow infection control and safety policies and procedures. 33. Complete resident incident reports per facility policy. 34. Coordinate and gather information necessary for resident monthly charges. 35. Order nursing, resident and housekeeping supplies as needed. 36. Read communication book daily and conduct follow-up activity as needed. 37. Ensure that medication carts are kept neat and clean. 38. Conduct monthly cart audits as assigned and take corrective action as needed. 39. Call in replacement staff when call-offs occur to ensure proper staffing. 40. Coordinate and schedule resident appointments with ancillary service providers as needed. 41. Coordinate and schedule resident transportation to ancillary service provider appointments as needed. 42. Participate in the training of new staff. 43. Ensure that residents’ medical records are kept neat and orderly. 44. Report medication errors to supervisor immediately. 45. Work collaboratively with Sales, Administration and nursing team to complete resident assessments on potential new admissions. 46. Initiates DOH reportable incidents in coordination with the supervisor 47. Participates in On-Call 48. Assists with annual staff competencies 49. Becomes Medication Trainer/Practicum Observer to evaluate and assist with Medication Tech trainings and competencies 50. Performs audits necessary to keep compliance in the day to day routines51. Conduct all other duties as assigned by supervisor and/or administrator.
