About The Position

Sodexo is seeking a Full-Time Wellness Dietitian to join their Clinical Nutrition Team at Mt. St. Antonio Gardens in Pomona, California. This prestigious 5-star CCRC spans 38 acres with nearly 500 residents and has a 40-year partnership with Sodexo. The role involves 60% direct patient care and 40% wellness engagement and community outreach, focusing on enriching the lives of seniors through nutrition and compassionate care within a close-knit, EDEN-trained team and an affluent community of residents.

Requirements

  • Registered Dietitian credentials
  • SNF or healthcare nutrition experience required
  • Strong knowledge of clinical documentation and regulatory standards
  • Collaborative, team-oriented approach with strong communication skills
  • Passion for working with seniors
  • Master's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) OR Bachelor's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) for those eligible for the registration exam prior to 1/1/24
  • Meets eligibility requirements for Registered Dietitian Nutritionist (RD/RDN) by the Commission on Dietetic Registration (CDR) and will become registered within 120 days of hire. In some states and/or facilities the RD/RDN credentials is required at the time of hire
  • Certification/licensure as required by state(s) of practice. In some states and/or facilities state certification/licensure is required at the time of hire
  • Minimum Management Experience - 1 year
  • Minimum Functional Experience - 2 years work experience in health care including hospitals, nursing homes, assisted or independent living facilities, long-term care, retirement homes, or clinics + registered dietitian

Responsibilities

  • Provide clinical nutrition care primarily within a 40-bed SNF, with occasional support across LTC, assisted living, and independent living
  • Consistently meet DOH/state/federal and company standards
  • Serve as an active member of the interdisciplinary team, working closely with nursing & medical staff and attending care conferences as assigned
  • Perform quality improvement initiatives such as test trays, food service audits, and meal rounds
  • Provide wellness engagement opportunities to the residents and employees as assigned
  • Supervise 3 direct reports, consult with independent living residents, and collaborate with onsite CDM
