About The Position

Valley Health at Home by BAYADA is currently seeking a Weekend Registered Nurse, RN for an opportunity to perform home health visits with adult and senior patients. This office works with clients located throughout the Las Vegas Valley. Please note: current / recent experience in a Medicare Home Health setting with OASIS documentation is required for this position. As a home health RN, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home. How You’ll Make an Impact: Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care) Triage and manage urgent clinical situations independently in the home setting Perform medication reconciliation , teaching, and safety monitoring Perform comprehensive wound assessment, care, and documentation What Will Make You Successful: Ability to work independently while knowing when to escalate concerns Comfortable making real-time clinical decisions without immediate on-site support Strong problem-solving skills in unpredictable home environments Ability to manage a dynamic visit schedule and prioritize urgent visits Efficient with point-of-care documentation and technology Strong attention to detail while maintaining productivity expectations Comfortable providing care in varied home environments Flexible and adaptable to changing patient needs and schedules Demonstrates cultural sensitivity and respect for patient autonomy

Requirements

• A current nursing license in good standing in the state of Nevada
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).
• current / recent experience in a Medicare Home Health setting with OASIS documentation is required for this position
