Job Description Weekend Intake Coordinator Schedule: Saturday and Sunday 8a-7p Demonstrate commitment to the Paradigm Health mission, vision and core values through your daily job performance. Support the business development team in obtaining required information & documentation for incoming referrals. Monitor faxes and other intake sources for incoming referrals and other referral related information. (Forcura, Epic, Allscripts, Cerner, etc.) Enter patients in EMR system (Wellsky), create cases in intake software (Forcura) and ensure information flows over. Enter new physicians, and referral sources as necessary. Initiate communication with business development team for all incoming referrals. Demonstrate initiative in requesting additional documentation as needed to support eligibility, proper coding and accurate billing in accordance with home health and hospice Medicare regulations. Demonstrate a sense of urgency and persistence in reaching physicians to collect necessary information and reaching patients/POAs to get referrals started on service. Maintain good verbal and written communication with team and/or referral sources on referral status including documenting status and progress in Forcura comments and updating trackers. Requirements

Medical Office experience is required
Proficient in Microsoft excel
Flexible, organized, and exercises sound judgment.
Meticulous attention to detail, chart accuracy, and compliance.
Excellent time-management.
Strong communication (written and verbal) skills
