Requirements

  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home health preferred.
  • Currently licensed as a Registered Nurse in the State(s) of planned practice.
  • Possesses and maintains CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Self-directed and able to work with minimal supervision.
  • Capable of prolonged or considerable walking or standing.
  • Able to lift, position and/or transfer patients.
  • Able to lift supplies and equipment.
  • Capable of considerable reaching, stooping bending, kneeling, and/or crouching.
  • Possesses visual acuity and hearing to perform required nursing skills.

Responsibilities

  • Conducts comprehensive in‑home admission assessments for new home health patients, including physical assessment and detailed health history, in accordance with regulatory and agency standards.
  • Completes all admission‑related documentation, including OASIS and the initial individualized plan of care, based on the comprehensive assessment and patient goals.
  • Verifies patient eligibility, homebound status, medication lists, and clinical information necessary to initiate home health services.
  • Collaborates with the certifying medical provider to establish admission orders and the initial plan of care; obtains additional orders as needed.
  • Provides initial patient and caregiver education on the plan of care, safety, medications, agency services, and expectations for ongoing home health visits.
  • Identifies and communicates urgent or significant changes in patient condition to the medical provider and appropriate agency staff.
  • Ensures timely, accurate, and compliant documentation of all admission activities within agency and regulatory timeframes.
  • Coordinates handoff communication to the weekday clinical team to ensure smooth and complete continuity of care following the weekend admission.
  • Ensures necessary equipment, supplies, or services required at the start of care are arranged or communicated to the appropriate team members.
  • Maintains thorough knowledge of applicable federal, state, and home health regulations and adheres to all agency policies and procedures.
  • Participates in required staff meetings, trainings, and orientation activities as assigned.
  • Maintains professional competence through required annual education and participation in ongoing training opportunities.
  • May be required to visit patients in environments that vary in cleanliness, ventilation, or safety, and must be able to travel in varied weather and traffic conditions.
  • Performs other admission‑related duties as assigned and appropriate to the scope of the Weekend Admission RN role.
