About The Position

As Treasure Coast Food Bank’s WCC Advisor, you will serve as a trusted guide and advocate for families working toward greater stability and self-sufficiency. You will partner with families to develop individualized Whole Child Plans, connect them to critical community resources, and assist with public benefit applications such as SNAP, Medicaid, TANF, and Florida KidCare. You will monitor progress, document outcomes, and help remove barriers that prevent families from accessing essential services. In addition, you will oversee intake and eligibility screening, and reporting for the Home Delivery Program, maintain accurate case records within required systems, and analyze performance data to support program impact. Through outreach and collaboration with community providers, you will help strengthen a coordinated network of services that advances food security and economic stability across the Treasure Coast.

  • Ability to provide services at off-site outreach locations as needed.
  • Have a valid FL Driver’s License and the ability to utilize a personal vehicle to travel between the organization’s service areas.

Responsibilities

  • Facilitate completion of the Whole Child Profile and guide families in developing a comprehensive Whole Child Plan.
  • Connect families to appropriate community services, public benefits, and support providers.
  • Advocate on behalf of families to ensure timely and responsible service delivery.
  • Monitor and document outcomes to evaluate progress and provider responsiveness
  • Conduct follow-ups with families and service providers to ensure accountability and plan effectiveness.
  • Revise service plans as needed to reflect changing family needs.
  • Assist families with applications for Medicaid, SNAP, TANF (Cash Assistance), and Florida KidCare.
  • Verify documentation and monitor application status through eligibility determination.
  • Serve as liaison between families and public assistance agencies.
  • Provide guidance through appointment scheduling and documentation submission.
  • Manage intake, eligibility screening, and reporting for the Home Delivery Program
  • Maintain accurate and timely documentation within required systems.
  • Analyze performance data to support program evaluation and continuous improvement.
  • Assist community partners in maintaining accurate resource directory information.
  • Represent Treasure Coast Food Bank at outreach events to promote Whole Child Connection services.
  • Educate families on navigating internet-based systems and benefit platforms.
  • Perfom other responsibilities as assigned in support of program operations and organizational goals.
