About The Position

UPMC Western Maryland's Hospice team is looking for a Volunteer & Bereavement Coordinator! This is a full-time, 40 hour/week position, working mostly Monday - Friday 8:00 am - 4:30 pm. Will be expected to work one evening a month (5:00 pm - 7:00 pm) to hold a bereavement support group and will have the honor of hosting a program to honor patients that passed away one evening annually. The Volunteer & Bereavement Coordinator will coordinate the activities and responsibilities of Hospice Volunteer and related Hospice activities as well as the delivery of bereavement services to family members/caregivers of Hospice patients. This will be done in a manner consistent with efficiency, cost effectiveness and regulatory compliance.

Requirements

  • Bachelor's Degree in Psychology, Counseling, Social Work, or related field required.
  • Competency in computer skills in a Windows based environment.
  • Demonstrate ability to perform computer application(s) as needed.
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

Nice To Haves

  • One year of experience in a related position preferred.

Responsibilities

  • Utilize innovative approaches to recruit and retain volunteers, enhancing and maintaining the Volunteer program.
  • Assign volunteers to hospice patients in a timely manner based on identified needs of the patient(s) and the volunteer's capabilities, ensuring the compatibility of the volunteer and patient/caregiver.
  • Assure the volunteer has the needed information necessary to meet the needs of the patient/caregiver.
  • Provide training and on-going education to ensure that volunteers are adequately prepared for their assigned job duties, developing specialized training for volunteers providing care to specialized populations.
  • Ensures that the volunteer is following the plan of care and meeting the identified needs of the patient/caregiver.
  • Communicates with the assigned volunteer when there are changes in the patient's condition, level of care and upon patient's death.
  • Oversee that all volunteers are up to date with all regulatory requirements as stipulated by Medicare CoP's, DOH, and UPMC/HNA and maintains Hospice Volunteer records in accordance with established policies, certifications, and accreditation regulations.
  • Facilitate regularly scheduled volunteer meetings and evaluate performance and competency of each active volunteer annually.
  • Evaluate and assess the bereaved? s grief needs and risk factors associated with grief, assessing potential risk of suicide and/or potential for abuse or neglect.
  • Assess environmental resources and obstacles to maintain safety. Assess special needs related to cultural diversity including communication, space, role of family members and caregivers, and special traditions. Communicate and document observations, services and evaluations through reports, progress records, and conferences.
  • Assess needs for community bereavement support groups relevant to targeted focus groups and demographic regions. Develop, facilitate and assess model(s) for bereavement support groups. Train and oversee, in conjunction with Hospice Volunteer Coordinator(s), any volunteer facilitating bereavement support groups.
  • Develop &/or make available bereavement educational material for families, staff and volunteers. Provide education to staff, volunteers and community related to grief, loss and bereavement. Plan, coordinate and implement memorial programs through collaborative efforts of the IDT. Participate in orientation, continuing education programs, clinical conferences, quality assurance and performance improvement activities.
  • Participate in Interdisciplinary Team conferences, attend department meetings, and meet with Supervisor.
  • Maintain and protect patient and Agency confidentiality. Promote personal and co-worker safety during work duties. Represent and participate in Agency professional and community activities, as requested.
  • Endeavor to achieve and maintain quality customer satisfaction in the delivery of services to internal and external customers. Demonstrate support of Agency mission and philosophy in delivery of services.
