Position Summary: This clinical position provides assertive community treatment services to clients assigned to the Assertive Community Treatment (ACT) program who have a severely persistent mental illness. These services are provided to assist in resolving or minimizing the effects of a client's impairment through a wide range of skills, including assessment, monitoring, crisis intervention, education, linking and training in community resources, assistance in daily living skills, and obtaining necessary services for meeting basic human needs and entitlements. This position serves as a consultant to the team in the area of vocational services.

Requirements

  • Bachelor's degree or a QMHS 3+ with high school diploma required.
  • LSW or LPC preferred.
  • Must have at least one year of experience in vocational rehabilitation services.
  • Specialized training in vocational rehabilitation from an accredited program may be substituted for one year of work experience.

Responsibilities

  • Provides assertive community treatment services to clients assigned to the ACT Team in accordance with the State mandates and established Agency and program policies, procedures, philosophy and client individual service plan.
  • Documents all services provided and maintains records utilizing the electronic record in accordance with agency policy/procedure.
  • Maintains client confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures.
  • With supervisors written approval provided the employee possesses the appropriate licensure, may perform diagnostic assessments and individual/group therapy under the direction of an independently licensed clinician.
