About The Position

Mass General Brigham’s Home Hospital Virtual Command Center is growing. We are seeking experienced Registered Nurses with at least 3 years of Emergency Department experience who are ready to transition from traditional bedside care into an innovative virtual care model. As a Virtual Registered Nurse (VRN), you’ll act as the “call light” for our Home Hospital patients—providing real-time clinical support, triage, and care coordination from our centralized command center. This role offers the pace, critical thinking, and clinical complexity of acute care, without the physical demands of direct patient-facing care. We provide hospital-level care for a diverse adult patient population, including: •Acute medical conditions such as heart failure, COPD exacerbations, pneumonia, and cellulitis •Dehydration requiring acute intervention •Postpartum patients with hypertension Available Schedules: • Full-time (36 hours): 7:00pm – 7:30am • Full-time (36 hours): 9:00pm – 9:30pm • Part-time (24 hours): 7:00pm – 7:30am This on-site role is based in our Logistic Command Center at Assembly Row in Somerville, MA—a high-tech, fast-paced, and innovative environment dedicated to delivering safe, evidence-based care in the home. You’ll also benefit from a self-scheduling model, with schedules built 4–6 weeks in advance to support work-life balance and flexibility. Job Summary Our Virtual Registered Nurses provide high quality, patient-centered, nursing care to patients across the continuum of services provided by Healthcare at Home. The VRN will work with a robust interdisciplinary team to promote highest patient outcomes. This role will utilize remote patient monitoring, assessment, planning, intervention, and critical thinking skills to care for patient populations in the home. Leveraging the resources and expertise of the MGB team to implement collaborative workflows, standards, policies, protocols, guidelines, and documentation systems to support safe, reliable, high-quality evidence-based care within clinical protocols as the foundation.

Requirements

  • Graduation from an accredited school of nursing.
  • Current license to practice as a Registered Nurse in Massachusetts
  • At least 3 years of Emergency Department experience required
  • Excellent communication, interpersonal, and organizational skills required

Nice To Haves

  • BSN preferred.
  • Telemetry experience preferred
  • Telehealth and remote patient monitoring experience preferred
  • Telephone triage experience preferred
  • Proficiency with Epic, Microsoft Outlook, Microsoft Teams, and other web-based platforms preferred
  • Experience working in a start-up environment and/or willingness to adapt to continuous change encouraged

Responsibilities

  • Performs and documents nursing process, assessment, diagnosis, planning, implementation, and evaluation of patients receiving care remotely, in their defined home.
  • Performs virtual care, patient assessment, remote patient monitoring (as ordered) and works with the clinical care team to provide care, trend data, and respond accordingly.
  • Provides protocol driven assessments, care coordination and intervenes as appropriate and defined by MGB Healthcare at Home protocols.
  • Exhibits sound clinical decision making and critical thinking skills for a diverse patient population.
  • Maintains and practices in accordance with nursing guidelines, as well as institutional, local, and state regulations.
  • Performs admissions, discharges, and episodic visits within MGB Home Hospital, through high quality virtual patient-centered care.
  • Performs triage of patient care calls outside of regular business hours for post-acute home care patients; requests a home care in-person visit as appropriate by the home care on-call nurse.
  • Serves as a role model for professional nursing practice.
  • Utilizes nursing knowledge to identify, prevent and or solve complex acute and/ Shares expertise with multidisciplinary teams to serve as a staff resource.
  • Supports research efforts to advance knowledge and promote evidence-based practice.
  • Performs other duties and responsibilities as assigned by Nursing Leadership.
  • Provides medication teaching, assists with medication administration and education to patients and families. Collaborates closely with pharmacy and care team regarding medication reconciliation process.
  • Completes timely documentation for all nursing care in Epic.
  • Answers, directs, and provides follow up on incoming clinical patient concerns
  • Ensures compliance with reporting requirements in RL Solutions.
  • Participates in quality and safety education, rounds, and evaluations.
  • Promotes professional practice and a culture of safety; engages in process improvement efforts.
  • Must be able to work in a HIPPA compliant environment.
