About The Position

VEN Centers, Powered by Arizona Liver Health (ALH), is a medical practice leading the charge in combatting the elimination of viral hepatitis in Arizona. With our extensive expertise in hepatology and a deep-rooted passion for our field, we adopt innovative strategies to bring Hepatitis C virus (HCV) treatment and cure directly to our patients, removing obstacles and establishing a new benchmark in healthcare VEN Centers is looking for qualified candidates to fill their VEN(Viral ElimiNation) Centers Advanced Practice Provider (APP) position. Successful candidates possess a passion for patient outreach and advocacy with the goal of delivering the best care and removing obstacles to provide necessary treatments to affected patients. This role will be pivotal in removing patient obstacles to treatment options and will require hands on efforts to ensure that patients are provided the care that they need. The Provider will primarily see patients from the Outreach team in a virtual capacity. Provider will be an integral part of SOP development for our program as well as staff training and education. Providers that have dedicated experience serving the LGBTQ community with HIV prevention would be a great fit for this role. We can train you on the hepatitis C component. This is a Hybrid / Remote role with some travel expectations to our sites around the state of Arizona. This is a full-time role based in the Tucson Metro Area (open to Southern AZ) with occasional travel to our pop-up clinic locations. The role is hybrid with flexibility for remote / in-office work.

Requirements

  • Proficient in use of Microsoft applications such as Outlook, Word, Excel, PowerPoint, and OneDrive as well as practice’s EMR system (eCW)
  • Excellent verbal and written communication skills
  • Strong ability to identify issues with “quietly unhappy” patients
  • Refers patients appropriately
  • Demonstrates awareness of quality-of-care metrics and regularly meets them
  • Solid interpersonal skills and ability to maintain good working relationships with peers, coworker, and patients
  • Ability to provide patient care with minimal supervision
  • Ability to perform in-clinic procedures (such as fibroscans) independently
  • Autonomous in call-rotation
  • Ability to diagnose and make treatment decisions for patients
  • At least one (1) year experience working in a clinic or hospital setting as an NP or PA with an Infectious Disease focus
  • Commitment to earning additional certifications/professional titles outside scope of work
  • Ability to resourceful and consults resources before asking others for instruction
  • Diagnoses and manage clinically complex patients with minimal assistance
  • Bachelor's Degree in Nursing and Nurse Practitioner or Physician Assistant Program from an accredited institution
  • Certification as a Nurse Practitioner with current RN and NP License or
  • Graduate of a university program approved by the Accreditation Review Commission on Education (ARCE) for the Physician Assistant with certification by the National Commission on the Certification of Physician Assistants (NCCPA), license to practice in Arizona
  • Must maintain certifications as required by department or division
  • Must have valid documentation and authorization to work in the U.S. for any employer

Nice To Haves

  • Previous outpatient hepatology/gastroenterology experience as an NP or a PA preferred
  • Bilingual (English / Spanish) preferred
  • Dedicated experience working on community HIV prevention preferred

Responsibilities

  • Provide direct patient care and clinical management for individuals with HCV, HBV, HIV, STI, and those on PrEP- Additional disease diagnoses as needed.
  • Conduct comprehensive assessments, including medical history, physical examinations, and laboratory evaluation.
  • Develop and implement individualized care plans, monitor treatment progress, and adjust interventions as necessary.
  • Educate patients on HCV, HBV, HIV, STI, and PrEP, including transmission, prevention, medication adherence, and lifestyle modifications.
  • Collaborate with a multidisciplinary team to ensure coordinated and holistic patient care.
  • Contribute to the development and enhancement of HCV, HBV, HIV, STI and PrEP care programs.
  • Identify areas for improvement in patient care and processes and propose and implement solutions.
  • Assist in the development of educational materials and training programs for healthcare professionals and community members.
