Urologist Job Quick Facts: Specialty: Urology Job Type: Locum Tenens Facility Location: Silverdale, WA Service Setting: Inpatient/Outpatient Reason for Coverage: Supplemental Coverage Period: ASAP - Ongoing Coverage Type: Clinical + Call Shifts needed to be filled: - Apr 20 -24, (weekend calls) 18 -19, 25 -26 - May 4 -8 - Jun 22 -26, Jun 29 - Jul 3 - Jul 6 -10, 13 -17, 20 -24, 27 -31 - Aug 3 -7, 10 -14, 17 -21, 24 -28, Aug 31 -Sep 4 - Ongoing availability Shift Schedule: 8a -4p Call Schedule: 5 -6 days of call - Nights: 5p -8a - Weekends: 7a -7a Call Type: Beeper Call Ratio: 1:6 Callback Volume: 80% callbacks, 20% consults Patient Volume: 20 -25 per day # of Surgeries: 20 -30 surgeries/month Rounding included: Yes Duties & Procedures: - bread & butter/General uro care (procedures, in -clinic, & on -call) - Cystoscopy, Ureteroscopy, Nephrectomy, Cystectomy - Prostate biopsy TURP, Vasectomies - Uroflow/cuff, Bladder scans - ESWL - Botox injections Support Staff: 2 RNs, MA, surgery scheduler, & 2 front office staff Trauma Level: III Office Equipment: Robot available EMR: Epic Other info: - Solo coverage - Hospitalists will handle admissions Travel, lodging, and malpractice insurance covered

- BC/BE
- Active WA License or IMLC
- DEA
- ACLS, BLS
- Self -Query NPDB report
