About The Position

VNA Hospice NWI is Hiring: Full-time, Triage RN for Home Hospice $65,000-80,000 Based on previous experience Every other weekend: Friday 4:30 pm- Monday 8:00 a.m., plus Two alternating shifts during the weekday 4:30pm-8:00 a.m. Coverage Area: Porter, LaPorte, Starke, Jasper, Newton and Lake County, mileage paid for visits made. The triage nurse is responsible for supporting patients, families, and all other callers by providing professional nursing support via the phone while also dispatching on-call staff and other members of the hospice care team to provide in person support when needed. Coordinates and prioritizes all service delivery after hours including patient and family education, dispatching of interdisciplinary visits, and coordination of services with external vendors and resources as needed. Provides initial bereavement support to families as needed. Documents interactions with patients, families, contacts, and vendors in the patient record to include the assessment, caller agreement to the plan, any actions and interventions and the resolution of each patient/family interaction. Promotes a positive customer service-oriented approach by being a calming presence and supporting voice for patients and family members in their time of need. The triage nurse serves as the first line of support for those needing assistance outside of regular business hours and will initiate intake process as needed to ensure streamlined access. Supports staff members and works in the field as needed. Initiates the complaint process as needed per organization’s policy. The triage nurse reports directly to the supervisor on call.

Requirements

  • Must have and maintain license as a registered nurse by the state of Indiana.
  • Minimum of associate degree in nursing, graduate of a school of professional nursing.
  • Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home came preferred.
  • Must be sensitive to issues of loss and grief.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid Indiana driver’s license and auto insurance.

Nice To Haves

  • Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred.
