About The Position

The Triage Care Advisor Registered Nurse supports BMG ambulatory primary care and specialty practices through triaging calls and coordinating care as needed. The Triage Care Advisor Registered Nurse serves in an expanded health care role to collaborate with the primary care practices, specialists, and other members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services. The Triage Care Advisor Registered Nurse follows evidence-based clinical guidelines, preventive guidelines, and protocols in care delivery to the patient. The Triage Care Advisor Registered Nurse coordinates care across acute and other ambulatory settings and helps patients/families understand heath care options.

Requirements

  • Licensed Registered Nurse in good standing for the State of Michigan

Nice To Haves

  • Bachelor degree (BSN or BS) preferred and two to five years of experience in nursing with adult and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing, or hospital based medical-surgical setting.
  • Experience in ambulatory care is desirable experience as well as acute care or post-acute care setting.

  • Regularly communicates both orally by telephone, and in writing with patients. Also communication with physicians, and other healthcare professionals to exchange information pertinent to their information provided.
  • Demonstrates effective problem-solving and critical thinking in care for patients.
  • Knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education.
  • Connects patients with Acute Care Managers and other resources as needed.
  • Critical thinking skills and ability to analyze complex data sets.
  • Ability to manage complex clinical issues utilizing assessment skills, resources, and protocols.
  • Excellent assessment and triage skills.
  • Ability to implement evidence-based interventions and protocols for chronic conditions.
  • Flexibility to adapt to the needs of the department
  • Knowledge of health concerns or changes in current health status and how they may affect patients in all age ranges from neonate through geriatrics for various medical specialties.
