About The Position

Treatment Nurse (LVN) – Weekend Only Willow Creek Health Care Center Pay Range: $28.00 – $30.00 per hour (DOE) Willow Creek Health Care Center is seeking a dedicated and compassionate Treatment Nurse (LVN) to join our team for weekend coverage. The Treatment Nurse is responsible for providing skin care treatments, wound care, and assisting in the development and implementation of resident care plans in accordance with current federal, state, and local regulations and facility policies. General Purpose The primary purpose of the Treatment Nurse position is to provide direct nursing care related to skin care, wound care, and treatments, and to ensure that the highest degree of quality resident care is maintained at all times. Requirements

  • Current, active LVN license in the state of California.
  • Experience in wound care and treatment nursing in a skilled nursing facility preferred.
  • Knowledge of skin care protocols, infection control, and documentation requirements.
  • Ability to work independently and as part of an interdisciplinary team.
  • Strong documentation and communication skills.
  • Current CPR certification required.
  • Experience with PointClickCare (PCC) preferred.
  • Must be able to move throughout the facility during the shift.
  • Must be able to lift, push, pull, and move up to 25 pounds.
  • Must be able to perform treatments and resident care tasks that may require bending, standing, and walking for extended periods.
  • May be required to assist in emergency situations.

Responsibilities

  • Make written and oral reports/recommendations to the attending physician, Medical Director, or Director of Nursing concerning the status and care of residents.
  • Maintain confidentiality of all resident protected health information and report any suspected violations.
  • Report any known or suspected unauthorized attempt to access the facility’s information systems.
  • Work with the Interdisciplinary Care Plan Team in developing comprehensive assessments and care plans for assigned residents.
  • Provide written and/or oral status reports on residents receiving treatments.
  • Evaluate and implement recommendations from committees as they relate to resident skin care and treatments.
  • Maintain effective working relationships with physicians, consultants, staff, residents, and family members.
  • Identify, manage, and treat skin disorders and lesions including abrasions, pressure injuries, ulcers, infections, and other dermatologic conditions.
  • Perform skin assessments using observation, inspection, and palpation techniques.
  • Complete quarterly dermatologic assessments and documentation.
  • Provide therapeutic services and treatments as ordered by the physician or Medical Director.
  • Ensure residents with pressure injuries receive appropriate prevention and treatment interventions.
  • Provide direct nursing care in emergencies and assist in admissions and discharges as needed.
  • Assist with rehabilitation activities including range of motion, ambulation assistance, and monitoring therapy progress.
  • Participate in care planning and in-service training programs related to skin care and wound care.
  • Monitor inventory and ensure adequate skin care and treatment supplies are available.
  • Follow infection control and safety procedures, including proper use of personal protective equipment (PPE).
  • Report occupational exposures, incidents, grievances, abuse allegations, or safety concerns in accordance with facility policies.
  • Ensure resident rights are maintained, including honoring refusal of treatment in accordance with advance directives and facility policy.
