About The Position
Heartline Staffing, a Strategic Talent Partner with Purple Cow Recruiting, is seeking an experienced Registered Nurse – Case Manager for a travel assignment supporting an acute care hospital in Mission Hills, California. This role focuses on inpatient care coordination and discharge planning for medically complex patients in a hospital setting. POSITION SUMMARY: The Registered Nurse – Case Manager is responsible for coordinating patient care across the continuum, facilitating safe and timely discharges, and collaborating with interdisciplinary teams to ensure optimal patient outcomes. This role requires strong acute care hospital experience in utilization review, discharge planning, and care coordination. ASSIGNMENT DETAILS: • Start Date: ASAP • Assignment Length: 13 weeks • Shift: Day shift, 5x8s (8:00 AM – 4:30 PM) • Weekend Requirement: Rotating weekends to meet department needs • Floating: Not required • Patient Ratio: 1:20 • First-Time Travelers: Accepted (clinical review required) • RTO Restrictions: No more than 7 days • License: Active California RN license required (pending accepted but must be active by start) COMPENSATION: • Regular Hourly Rate: $24.00 • Overtime Rate: $36.00 • Weekly Taxable Pay: $960.00 • Weekly Travel Stipend: $1,792.00 • Total Weekly Compensation: $2,752.00 WORK ENVIRONMENT: • Acute Care Hospital • Unit: Case Management • Scrub Attire: Scrubs or Business Attire • Charting System: Epic • Patient Population: Adolescents, Adults, Geriatrics • Strong interdisciplinary support including Social Services and Transportation
Requirements
- Active California RN license required (pending license accepted but must be active by start date)
- Minimum 2 years of acute care hospital RN Case Manager experience REQUIRED
- MUST have hospital-based care coordination and discharge planning experience
- Epic charting experience required
- Ability to manage a caseload of approximately 1:20 patients
- Weekend rotation availability required
Nice To Haves
- Experience using InterQual criteria preferred
- Strong knowledge of CMS guidelines and DRG workflows
- Travel experience preferred; first-time travelers accepted with clinical screening
