Responsibilities

  • Lead and verify medication therapy management for diverse patient populations, including reconciliation, optimization of regimens, and prevention of clinically significant interactions.

  • Oversee formulary stewardship, collaborate on clinical pathways, and participate in medication safety initiatives that elevate care quality.
  • Support immunization services, anticoagulation programs, and transitions of care, contributing to measurable improvements in outcomes.
