As a licensed practical nurse in a long-term care setting, you’ll play a vital role in delivering safe, patient-centered care that respects residents’ dignity, autonomy, and comfort. You’ll collaborate closely with registered nurses, physicians, therapists, social workers, and families to ensure comprehensive, coherent care plans that address physical, emotional, and psychosocial needs. Responsibilities

  • Provide direct nursing care to long-term care residents, completing assessments, administering medications, and monitoring health status per scope of practice.
  • Administer treatments, manage wound care, perform perioperative and post-acute tasks as applicable, and assist with activities of daily living, mobility, and rehab-supportive therapies.
  • Document accurately in the electronic medical record, update care plans in partnership with RNs, and participate in interdisciplinary rounds to optimize patient outcomes.
  • Monitor vitals, respond to changes in condition, recognize red flags, and implement appropriate interventions while adhering to clinical protocols and safety standards.
  • Support admissions, transfers, and discharges, ensuring smooth transitions and clear communication with families and care teams.
  • Provide patient and family education on medications, care routines, safety, and disease management; advocate for residents’ preferences and goals of care.
  • Maintain infection control practices, fall prevention strategies, and safe medication administration, while complying with regulatory requirements and facility policies.
  • Document and report incidents, participate in quality improvement initiatives, and assist with training and orientation for new staff as needed.
  • Engage in ongoing learning to stay current with LTC best practices, care innovations, and regulatory changes; seek opportunities for specialty certification and expanded responsibilities.
