Responsibilities

  • Provide high-quality, patient-centered nursing care to residents, including medication administration under physician order, routine assessments, and timely clinical documentation.
  • Monitor vital signs, observe for subtle changes in condition, and promptly communicate concerns to the interdisciplinary team to prevent complications and support continuity of care.
  • Assist with daily living activities, reinforce infection control practices, and help residents maintain independence and comfort within the facility’s care plan.
  • Collaborate with RNs, physicians, therapists, and social services to develop and implement individualized care plans, ensuring each resident’s physical, emotional, and psychosocial needs are met.
