Responsibilities

Provide high-quality, patient-centered nursing care to residents, including medication administration under physician order, routine assessments, and timely clinical documentation.
Monitor vital signs, observe for subtle changes in condition, and promptly communicate concerns to the interdisciplinary team to prevent complications and support continuity of care.
Assist with daily living activities, reinforce infection control practices, and help residents maintain independence and comfort within the facility’s care plan.
Collaborate with RNs, physicians, therapists, and social services to develop and implement individualized care plans, ensuring each resident’s physical, emotional, and psychosocial needs are met.
