About The Position

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Travel Licensed Practical Nurse (LPN) to join a Skilled Nursing Facility (SNF). Requirements

• Active LPN license (state or compact)
• Minimum 1–2 years of experience in a Skilled Nursing Facility (SNF) or similar setting
• Strong clinical skills and a commitment to patient care
• Ability to adapt quickly to new environments and travel assignments
• Diploma or Certificate in Practical Nursing from an accredited institution
• Basic Life Support (BLS) certification (AHA preferred)
• COVID-19 vaccination documentation (if required by facility)
• Valid government-issued ID

Nice To Haves

• Experience in long-term care, rehabilitation, or SNF settings
• Familiarity with Electronic Medical Records (EMR) systems

Responsibilities

• Deliver direct patient care in accordance with individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor patient conditions and report any changes to the RN or physician promptly
• Assist residents with activities of daily living (ADLs), including hygiene, mobility, and feeding
• Maintain accurate, timely, and compliant patient documentation
• Adhere to infection control protocols and safety standards
• Collaborate effectively with interdisciplinary healthcare teams to improve patient outcomes
