Responsibilities

  • Provide direct nursing care to long-term care residents under the supervision of a registered nurse, including initial and ongoing assessments, monitoring health status, medication administration within licensure, wound care, and treatment implementation.
  • Update and follow care plans, document accurately and promptly, and communicate changes to the care team.
  • Support activities of daily living, assist with mobility and transfers, promote safety and infection control, and help residents maintain dignity, independence, and quality of life.
  • Collaborate with physicians, therapists, social workers, and family members to coordinate holistic care, and participate in quality improvement and regulatory compliance activities.
  • Mentor and support Certified Nursing Assistants and new staff, modeling best practices in compassionate, patient-centered care.
