About The Position

The Trauma Intervention Specialist (TIS) is a key member of the team providing comprehensive social services support and long-term interventions that result in mental and emotional wellbeing in addition to the medical care to support physical wellbeing to victims of violent injury. The TIS is responsible for engaging youth and young adults (ages 14-30) who were treated at JEH or referred by a community partner after suffering a violent injury. The TIS is expected to conduct all visits in a trauma-informed manner and to model effective ways of engaging with resources to support healing. This job requires a significant amount of community work and a willingness to engage with a diverse range of neighborhoods in Philadelphia. The primary role of the TIS is to assist in engaging with clients during their involvement with TIP and to guide their treatment and recovery through the program. Job Description Provides acute and long-term therapeutic services and case management to Trauma Intervention Patients. Engage with young adults and their caregivers in the aftermath of a traumatic, violent injury. Accountable for developing and implementing an intervention care plan, in collaboration with clinical care team, using specific therapeutic techniques. Conduct bedside, , and office visits and phone outreach to patients to monitor their status, evaluate the effectiveness of the individualized plan of care, and identify new needs. May alter the individualized plan of care or case status based on the ongoing needs of the patient. Oversee the Community Health Worker Peers as they provide support and mentoring to patients and clients Accountable for thorough, timely and accurate documentation of all medical recordsand case management databases. Follow-ups with the patient according to established clinical program protocols and timeframes to monitor their status, evaluate the effectiveness of the individualized plan of care, and identify new needs Advocates on behalf of the patients/families to enable them to obtain necessary services. May include referrals to partner organizations where necessary.

Requirements

  • Master’s Degree in Social Work, Psychology, Counseling, or Family Therapy
  • At least 1 year of experience in a therapeutic setting
  • Relevant licensure (LSW, LCSW, LPC, LMFT) in Pennsylvania (preferred)
