About The Position

We are seeking a compassionate and dedicated Transportation Aide / CNA to join our healthcare team at Lone Tree Retirement Community. This vital role involves assisting patients with mobility and transportation needs within medical facilities, ensuring safe and respectful transfer of individuals across various care settings. The ideal candidate will possess a strong background in patient care, with experience in diverse healthcare environments such as hospitals, nursing homes, assisted living, or home care. Knowledge of medical documentation, EMR/EHR systems, and patient safety protocols is essential to provide high-quality support to our patients and staff. Duties Safely transport patients within hospital, clinic, or long-term care settings using appropriate equipment such as Hoyer lifts and wheelchairs. Assist with patient mobility including transfers, positioning, and ambulation while adhering to aseptic techniques and infection control standards. Support patient care activities such as vital signs measurement, medication administration assistance, and basic life support when necessary. Maintain accurate medical records and documentation using EMR/EHR systems like Cerner or eClinicalWorks. Communicate effectively with healthcare team members regarding patient needs and transfer status. Ensure compliance with HIPAA regulations to protect patient confidentiality. Assist with special care needs including Alzheimer’s/dementia care, memory care, behavioral health support, and working with individuals with developmental disabilities or disabilities of varying types. Operate equipment safely and maintain cleanliness of transfer devices and mobility aids. Provide compassionate support tailored to the needs of pediatric, adult, geriatric, or behavioral health patients across inpatient, outpatient, or home care settings. Qualifications Current CNA certification or relevant healthcare experience preferred; previous experience in hospital medicine, ICU, emergency medicine, or post-acute care is advantageous. Familiarity with medical terminology, anatomy, physiology, and aseptic techniques required. Experience working with vulnerable populations such as seniors, individuals with disabilities or developmental disabilities, Alzheimer’s/dementia patients, or behavioral health patients highly desirable. Proficiency in EMR/EHR systems like Epic, Cerner, eClinicalWorks preferred; familiarity with medical documentation standards including HIPAA compliance essential. Ability to operate patient transfer equipment including Hoyer lifts and wheelchairs safely. Strong communication skills and ability to work effectively within a multidisciplinary healthcare team. Physical ability to assist with patient transfers and mobility tasks; capacity to lift or move patients safely as needed.
