Sentara Heart Hospital located on the campus of Sentara Norfolk General Hospital in Norfolk, Virginia is hiring a Transplant Coordinator Registered Nurse for the Advanced Heart Program! The Transplant Coordinator plays a critical role in facilitating the complex logistics of organ transplantation while prioritizing patient safety and well-being while ensuring a smooth and efficient journey for both the donor and recipient. The Transplant Coordinator coordinates the clinical management of the transplant/VAD patient including the transplant/VAD evaluation, the transplant/VAD process and follow-up care. Provides clinical leadership to care team members, allowing staff to plan, direct, manage outcomes and evaluate care. Advanced Heart Failure Unit: 24 bed Specialty Unit that provides high acuity cardiac care to patients with severe heart failure, pre/post heart transplant, the Total Artificial Heart, and pulmonary hypertension. Work with high tech equipment such as mechanical circulatory support including ventricular assist devices (LVAD, BiVAD) and telemetry monitors. Environment relies on 24 hours consistent communication and intricate interdisciplinary teamwork skills. Nurses handle 3-4 patients on this unit based on acuity level.
