About The Position

The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.

Requirements

  • Bachelors Degree in Nursing or Associates Degree in Nursing
  • If Bachelors Degree: 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg or
  • If Associates Degree: 5+ years of acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg
  • MN Registered Nurse (RN) License
  • Basic Life Support (American Heart Assoc or Red Cross)

Responsibilities

  • Coordinate patient care in both pre- and post-transplant phases
  • Collaborate with interdisciplinary team members within the organization and outside community partners
  • Act as a liaison for transplant physicians to implement medical management of conditions
  • Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes
  • Apply appropriate medical interventions and transplant protocols based on patient assessment, laboratory results, and patient condition changes
  • Assumes on-call responsibilities after office hours by providing a resource for medical triage. Acts as transplant coordinator when donor organ becomes available.
  • Provides on-going education to patients and families
  • Collaborate with other members of the clinical team to improve patient care and benchmarks through quality improvement projects
  • Participate in the development and revision of protocols
  • Maintain knowledge of current practices in transplantation, nursing, and healthcare
