About The Position

In this role, you will implement effective case management strategies for an assigned member panel, providing oversight and assessing the health management needs of members. Your expertise will ensure that appropriate clinical interventions are identified and delivered. You will conduct outreach to members, develop personalized care plans, and coordinate care to help members achieve their health goals. Requirements

• High School/GED required; Bachelor's Degree in Nursing preferred.
• 7 years of clinical, case/utilization management, and/or health insurance experience required.
• 1 year of experience in a clinical setting required.
• Current State RN licensure or multi-state licensure.

Nice To Haves

• 5 years in Utilization Management/Case Management/Quality Assurance/Managed Care.

Responsibilities

• Maintain oversight over a specified panel of members by performing ongoing assessments of their health management needs.
• Implement review processes that comply with industry and regulatory standards within the care manager’s professional discipline.
• Create care plans for assigned members that address identified needs and remove barriers to care.
• Document all activities in compliance with business processes, regulatory requirements, and accreditation standards.
• Assist with other duties as assigned.
