About The Position

United Health Services (UHS) is seeking a dedicated Pharmacist to join our Transitions of Care Pharmacy team. The Pharmacist will ensure the safe and accurate administration of medications by verifying the clinical appropriateness of prescribed treatments and overseeing proper delivery methods, tby promoting patient safety and optimizing therapeutic oues. The Pharmacist is responsible for reviewingplex medication regimens, including prior to admission medications and discharge medications. The Pharmacist will work to ensure a smooth transition from hospital to home, including addressing adnce barriers such as medication affordability and transportation barriers. The pharmacist will also provide patient/caregiver education on medications and pertinent disease states.

Requirements

  • A Bachelor's of Science Degree (B.S.) or Pharm.D. in Pharmacy from an accredited college or university
  • Licensed as a Pharmacist in New York State or NYS internship permit

Responsibilities

  • Review medication orders for appropriateness (dose, route, frequency, indication, interactions, allergies)
  • Perform medication reconciliation upon patient admission, transfer, and discharge
  • Monitor patient therapy oues and adjust or rmend changes to optimize treatment
  • Provide drug information to healthcare providers, patients, and caregivers
  • Provide patient/caregiver education on medications and pertinent disease states
  • Evaluate and prevent adverse drug events and medication errors
  • Assess and address adnce barriers to medications prior to discharge
  • Document all activities in the electronic health record (Epic)
  • Ensuremunication and coordination between healthcare providers and patient/caregivers
  • Ensurepliance with state and federal regulations
