About The Position

The Pharmacy provides pharmaceutical services to patients across the continuum of care. Services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation and a meds to beds program for inpatients. Baptist Health Pharmacy services are committed to ensuring the optimal, safe, and effective use of medications for all patients through advanced technology and innovative application of skilled personnel that will improve clinical outcomes and community health in a value-based environment. Transitions of care pharmacists are responsible for target interdisciplinary programs and clinical services and ensures pharmaceutical care programs are appropriately integrated throughout BH system. In these clinical roles, transitions of care pharmacists participate in all necessary aspects of the medication-use system while supporting comprehensive and individualized pharmaceutical care in their assigned areas. They also serve as clinical resources and liaisons to other departments, hospital personnel, or external groups. In their role, they may conduct practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Transitions of care pharmacists will serve as subject matter expert for their area of care.

Requirements

  • Graduate of accredited College of Pharmacy
  • BS degree and one year Experience required.
  • Kentucky and Indiana Pharmacist License
  • Registration with Kentucky Board of Pharmacy (KY hospitals), within 30 days of hire date if you are a new registrant. If previously registered, you must have registration upon hire.
  • Basic Life Support Required.
  • New employees requiring CPR-Basic Life Support (BLS)-American Heart Association should receive (CPR) training within the first week of orientation.

Responsibilities

  • Clinical Services Management, Development, & Outcomes
  • Collaborates with physicians and healthcare team.
  • Communicates effectively and solves problems.
  • Provides medication therapy consultations and patient education.
  • Participates in activities to improve patient outcomes (MUE, ADR reporting, drug monitoring, resolving therapy issues).
  • Handles admission/discharge medication reconciliation.
  • Ensures safe care transition from inpatient to outpatient.
  • Supports Meds to Beds Program and outpatient therapy adherence.
  • Assists with prior authorizations and medication cost issues.
  • Assists with formulary management for safe, cost-effective medication use.
  • Acts as pharmacy specialist for complex cases, adverse drug events, and drug shortages.
  • Provides verbal and written medication consultations.
  • Stays current with pharmacy trends and shares updates.
  • Coordinates education, training, and competency assessments.
  • Precepts students and residents.
  • Attends conferences and workshops as needed.
  • Ensures pharmacy operations comply with policies, regulations, and safety standards.
  • Demonstrates commitment to Baptist Health values and quality practices.
