About The Position

Centurion is proud to be the provider of comprehensive healthcare services for the Indiana Department of Correction. We are seeking a full-time Regional Transitional Healthcare Facilitator to join our team providing support at correctional facilities and parole district offices throughout the Northern Indiana Region. Facilities include: Indiana State Prison - Michigan City, IN Westville Correctional Facility - Westville, IN South Bend Work Release - South Bend, IN Chain O' Lakes Correctional Facility - Albion, IN LaPorte Juvenile Correctional Facility - LaPorte, IN The Regional Transitional Healthcare Facilitator will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release as well as the coordination with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished. This Transitional Healthcare Facilitator will engage with recent parolees post-release to assist with and support the continuum of care, provide education and support to the released offenders and connect them to community resources to enable a successful transition into their community. The Regional Transitional Healthcare Facilitator will be assigned projects at and travel to correctional facilities and parole district offices as needed throughout our Northern Indiana Region.

Requirements

  • Bachelor’s degree in human services, social services, or criminal justice field
  • Two years of experience working in a correctional setting
  • Experience with/knowledge of community and state programs and services
  • Must maintain current tuberculosis screening documentation and active CPR and Automated External Defibrillator (AED) use certification
  • Must be able to pass background investigation and obtain agency security clearance where applicable

Nice To Haves

  • Two years of experience in re-entry services or release planning preferred
