Requirements

One year Hospice experience preferred
Current driver's license
Must have reliable transportation and insurance

Responsibilities

Excellent communication, negotiation and public relation skills.
Ability to market effectively with customers, referral sources, and the community.
Responding to customer and community needs regarding education as it relates to the hospice philosophy of care and delivery of services.
Developing and increasing the access to Paradigm Hospice & Palliative Services within the service area.
Assist, facilitate and coordinate between our referral sources, business development associates, our intake team and our patients and their families, the referral and admission process to promote a continuum of care model.
Facilitate the consent and referral to admit process.
Serve as a resource to review and provide available patient information related to a referral.
Ensure coordination and communication with referral sources and hospice agency leadership once consents are completed.
Serve as liaison between assigned hospital(s), skilled nursing facilities, community and hospice programs, with coordination of referrals/admission process and services.
Promptly respond to customer, both internal and external, and referral source requests and concerns
Compliance with accepted professional standards and practice.
Self-directed with the ability to work with little supervision.
Flexible and cooperative in fulfilling all obligations.
Ability to be creative and "think outside the box" and have fun doing so.
Great organization skills.
Develop relationships in the community on behalf of the company.
