The Transition in Place (TIP) Program is a U.S. Department of Veterans Affairs Grant and Per Diem (GPD) Program that places Veterans experiencing homelessness into apartments and works with Veterans to assume the lease within six to twelve months. This role will provide case management and supportive mental health and substance use counseling to a caseload of up to 15 Veterans. This role assists Veterans in their transition from homelessness to housing through permanent housing lease assumption. This position employs a variety of evidence-based interventions including Motivational Interviewing, Relapse Prevention and Trauma Informed Care to provide client centered services to Veterans.
