We are a transfer center, and capacity management team for all of UPMC Central PA. Through the transfer center, we offer advanced resources for all aspects of patient referrals, including urgent and non-urgent transfers, direct admissions, and provider-to-provider consultation. Our transfer center coordinators work directly with providers to document patient information and facilitate timely transfers. In addition, we partner with UPMC Community LifeTeam EMS (ground) and Stat MedEvac (air) to facilitate timely transports for our patients. This position will work/train in the office for up to the first 6 months depending on the individual. After training is complete this position has the possibility to work hybrid. Schedule: Night shift only. Anywhere from 10:45 p.m. - 7:15 a.m. or 6:45 p.m. – 7:15 a.m. Will work rotating weekends and holiday. Purpose: Serves as the primary contact for referring physicians requesting the services from UPMC in Central PA for continued patient care. Facilitates all patient transfers into and outgoing transfers, as well as all direct admissions for any service line from any source for all 7 UPMC Central PA hospitals. Facilitates bed placement for every patient in all 7 hospitals while operationalizing capacity management strategies for Central PA. Integrates with UPMC Community LifeTeam for timely and efficient patient movement. The position requires the ability to multitask, identify and escalate concerns, maintain great customer service, and foster the intersection for patient access to healthcare services via MedCall, the central control center for UPMC Central PA.
