The Time Limited Subsidy Case Manager, formerly known as the Rapid Rehousing Case Manager, plays a crucial role in providing services to homeless transition-aged youth (ages 18-24) under the guidance of the Program Manager. This position is focused on assessing the needs of these young individuals and connecting them to suitable housing options, which may include crisis or bridge housing, permanent supportive housing, shared living arrangements, or general affordable housing. The Case Manager is responsible for assisting youth in managing their lives and achieving their goals as outlined in their Individual Action Plan (IAP). This involves providing referrals for various services, including mental and physical health provisions, mainstream benefits, education, employment, and vocational training. The Case Manager will conduct initial intakes and needs assessments, utilizing tools such as the Next Step screening tool to enroll clients in the Time Limited Subsidy Program. They will manage a caseload of 20-25 youth, helping clients set personal goals and develop plans to achieve them through the IAP. Regular face-to-face contact is essential, with weekly meetings for unhoused clients and home visits once housing is secured. The Case Manager will also collaborate with partners to outreach to homeless youth in South Los Angeles and establish relationships with landlords and housing providers to create a portfolio of housing options for clients. In addition to housing support, the Case Manager will coordinate services in areas such as employment training, money management counseling, mental and medical health services, and legal and benefits counseling. Participation in coordinated case conferencing meetings with service providers is expected, along with the development and facilitation of workshops on various life skills topics. Data collection and entry into HMIS and Salesforce databases are required to track client progress and evaluate program outcomes. The Case Manager will maintain accurate client case files and submit monthly service reports to the supervisor, tracking client status and the supportive services provided. Attendance at required LAHSA meetings and relevant community meetings is also part of the role, as is acting as a liaison among various stakeholders to facilitate communication regarding client housing concerns.
