About The Position

Under the general direction of the Regional Clinical Manager, Family Services - Tissue, the Tissue Family Services Coordinator 1 (TFSC 1) is responsible for coordinating a wide range of support for potential tissue/eye donor families. Information is to be articulated in an easily understood and accurate manner to families whose ability to process information is impaired by emotionally traumatic circumstances. Infinite Legacy’s Tissue Family Services Coordinator 1 coordinates and facilitates emotional support during and after the discussion of the donation process. Rapport is established with donor families, Infinite Legacy team members, and hospital staff involved in the donation process. The Tissue FSC I will be aware and sensitive to diversity of all kinds (cultural, religious, educational, etc.) while working with families and hospital staff.

Requirements

  • Master’s degree at an accredited college or university in social work, counseling, psychology, nursing, or related degree. Bachelor’s degree will be considered if relevant experience is met.
  • 1- 2 years’ experience in an acute health care setting (preferred), hospice, bereavement counseling center, crisis intervention, etc. where there was active involvement in issues involving death, dying, grief and loss.
  • Ability to interact with grieving families in a supportive, empathic manner while taking into consideration actual or potential cultural diversities.
  • Ability to communicate effectively, verbally and in writing to provide information to donor families, recipients, health care personnel and the general public.
  • Ability to comfortably use technology to access and utilize Infinite Legacy databases, draft letters, send/receive email, and access the Internet for vital information/resources to assist donor families. Must have the ability to connect to the Internet safely with speeds capable of supporting both a VoIP phone and laptop. The requirements for such can change at any time and the user must be able to able to maintain current technologies.
  • Must be highly accountable and able to work autonomously. Must possess a high level of self-motivation and well-developed time management, organizational, and problem-solving skills.
  • The Per-Diem TFSC is scheduled on an as needed basis. This would include nights, weekends, holidays as needed to fill the schedule. The Per Diem employee would work minimum of one 12 hour shift a week.

Responsibilities

  • The Tissue FSC I will establish initial contact over the phone with the potential donor family and offer the option of tissue and eye donation in accordance with all state, federal and industry related regulations. Provides the family with a copy of an authorization or disclosure, Document of Gift if applicable and completes the medical/social history proficiently.
  • The Tissue FSC I will provide trauma, grief, and bereavement support services for the family over the phone during initial contact. Tissue FSC I will identify if additional support is needed and connect the family with local hospital or community resources to provide additional support. In addition, the plans for grief and bereavement services will be communicated to the Aftercare Team at Infinite Legacy who is responsible for following the donor family after donation.
  • Completes all necessary documentation within the time limits established by Infinite Legacy and its regulatory bodies.
  • Collaborates with local funeral homes and whole-body donation programs to ensure that the donor family is supported, and the funeral home or whole-body donation program is aware of the tissue recovery process.
  • Works in conjunction with appropriate Infinite Legacy staff during local tissue and eye cases. Collaborates closely with clinical personnel, DSC, Tissue AOC, HS, and TR during donor management to ensure that donor families’ needs are communicated to the Infinite Legacy team in addition to ensuring all case logistics are being communicated effectively to support donor and family needs during the donation process.
  • Triages phone calls from families that are received from the Donor Services Center as it pertains to tissue donation needs, hospice referrals and medical evaluation for donation. At times collaboration with Hospice RNs, Nursing Homes or Assisted Living Facilities are needed to ensure the donors wishes can be honored.
  • Navigates sensitive conversations as it pertains to the rejection of tissues post donation process. Initiates follow-up support over the phone to donor families as it relates to offering donor in spirit post donation should tissues be rejected or not able to be transplanted.
  • Facilitates communication with outside OPO’s who require local registry checks within IL’s DSA. This includes DC, VA, and MD registry checks in a timely manner.
  • Initiates follow-up support to donor and non-donor families when appropriate and is responsible for ensuring effective communication and documentation of aftercare components within the time limits established by Infinite Legacy and its regulatory bodies.
  • Participates in continuing education as identified by the Family Services Manager – Tissue. This could be but is not limited to conversation coaching, SIM, educational seminars, and classes.
  • Attend regularly scheduled Infinite Legacy staff meetings, FS team meetings, and case review.
