About The Position

The Therapy Team Lead supports the Director of Rehab with daily management and shall supervise the therapy team ensuring ethical and effective services based on best practices. Coordinates training and provides oversight of the Staff Therapists and Contract Therapists; monitors utilization, documentation and processes of therapists including but not limited to timely submission of documentation. Work with the DOR to lead and develop the service in line with patient needs ensuring that therapy input is integrated into the patients’ overall care plan.

Requirements

  • Licensed to practice as physical therapist, occupational therapist within the State, if applicable.
  • Two years of appropriate experience as a physical therapist or occupational therapist. Community/home health experience is preferred.
  • Ability to exercise initiative and independent judgment.
  • Acceptance of philosophy and goals of home health and hospice.
  • Demonstrates good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

  • Community/home health experience is preferred.

Responsibilities

  • Ensure Plans of care and goals are appropriate and reflected accurately in documentation.
  • Align patient’s diagnosis, needs and outcomes with the appropriate frequency of care, which meet standard of practice
  • Conduct chart audits to monitor and ensure excellent outcomes.
  • Provide education and coordination amongst staff to optimize accurate assessment of functional status, resulting in appropriate reimbursement.
  • Assists with communication and collaboration between Rehab and Nursing staff in effort to improve timely patient outcomes
  • Provide Starts of Care, Recertifications, and back-up patient therapy visits and visits as required.
  • Network across the continuum of care – acute, outpatient and long-term care – both amongst providers and direct care staff. Collaborate with other community providers to explore how PASCO/SW Therapy department can provide improved care, making our services more accessible and increase collaboration amongst providers.
  • Provide educational opportunities for both providers and caregivers – host virtual and in-person educational forums.
  • Provide support to other clinicians regarding effective caseload management and monitoring strategies.
  • Provide direct support and guidance to clinicians with potentially challenging cases/situations, particularly where reassessment may be required.
  • Provide and encourage others to share their skills/knowledge within both formal and informal environments
  • Responsibility for the development of therapy staff and support staff.
