About The Position

Nexus Family Healing is excited to add a part time hybrid Therapist for its new 180 Degrees partnership! This is a hybrid, shelter-based position based at the St. Cloud Youth Shelter with some work remotely at home. Located near McKinley Park, 180 Degrees St. Cloud Youth Shelter is the only 24-hour emergency shelter for youth under 19 in a large, 13-county region of Central Minnesota. Why This Work Matters: 180 Degrees' shelters support youth ages 12–19 who are navigating trauma, crisis, and major life transitions. Stays are brief — often just 14–30 days — but the impact can be life-changing. We’re looking for clinicians who bring creativity, compassion, and a willingness to meet youth where they’re at. Position Summary: The Community-Based Therapist provides mental health therapy to children, adolescents, adults, and families in homes, community settings, via telehealth, and at Nexus Family Healing offices. Using a trauma-informed, strength-based approach, the therapist delivers individualized care through evidence-based models like EMDR, DBT, TFCBT, and Systemic Family Therapy.

Requirements

  • Master’s degree in one of the behavioral sciences, human services, social work, marriage & family therapy, psychology, counseling, or clinically related fields required
  • Fully licensed in Minnesota as an LICSW, LMFT or LPCC
  • OR clinical trainee level with 5 years' shelter/residential experience
  • Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements
  • Must successfully pass a DHS criminal background check.

Responsibilities

  • Complete intake forms and diagnostic assessments.
  • Ensure timely and accurate clinical documentation.
  • Complete CANS assessments using TCOM principles.
  • Develop, update, and monitor individual treatment plans (ITPs) based on assessments and CAT/CANS.
  • Provide crisis intervention and safety planning as needed.
  • Participate in mandatory clinical supervision and team case consultation meetings.
  • Participate in family collaborative intake meetings.
  • Provide client-centered, strength-based therapy in collaboration with families.
  • Deliver individual, group, and family therapy aligned with treatment goals.
  • Meet direct service and documentation requirements.
  • Support discharge planning and post-care coordination.
  • Coordinate referrals and collaborate with families, schools, and providers.
  • Attend supervision, team meetings, and required trainings.
  • Maintain compliance with all clinical and regulatory standards
  • Communicate client needs with care teams, families, and community partners.
  • Ensure confidentiality and HIPAA compliance.
