Requirements
- Must be a graduate from an Accredited Therapy Program and have a Bachelor's Degree
- Must possess current licensure for MN, ND or IA
- Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care.
- Must be capable of performing the essential functions of the job, with or without reasonable accommodations.
- Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.
Responsibilities
- Assess & Evaluate Patients: Conduct comprehensive evaluations to identify physical, cognitive, communication, and/or swallowing needs based on discipline-specific scope of practice.
- Develop & Implement Treatment Plans: Create and carry out individualized treatment plans and rehabilitation programs to address functional deficits and promote independence.
- Provide Therapeutic Interventions: Deliver skilled, hands-on therapy and therapeutic activities, including exercises, manual techniques, adaptive strategies, assistive device training, and communication or swallowing interventions as appropriate.
- Educate & Support Patients, Families, & Staff: Provide education such as but not limited to pain management, fall prevention, home safety, coping strategies, communication techniques, and strategies to improve daily functioning and quality of life.
- Monitor Progress & Document Care: Track patient outcomes, document evaluations and progress, and modify treatment plans based on patient response and clinical judgment.
- Collaborate with Interdisciplinary Teams: Work closely with physicians, nurses, therapists, educators, and other healthcare professionals to ensure coordinated, comprehensive patient care.
