About The Position

The Therapeutic Interchange Pharmacist is responsible for providing day-to-day clinical operational needs with the interchange and dispensing of prescription medication as required in meeting the needs and requirements of a long-term care pharmacy, adheres to the company’s quality and customer service standards, policies and procedures, state and federal guidelines. SCHEDULE: Monday to Friday (3:30pm - 12am) with rotating weekends (10:30am - 7pm)

Requirements

  • Perform accurately and efficiently when dispensing prescription medications
  • Ability to work flexible hours, which may include evening, weekends, and holidays
  • Experience using Framework LTC
  • Experience using Microsoft Word, Excel and Microsoft Outlook
  • Excellent interpersonal skills and well-developed verbal and written communication skills with English as the primary language.
  • Ability to work independently as well as working in a team environment, meet deadlines, establish priorities, and be flexible, work under pressure with multiple tasks. Must have strong organizational, customer service, and time management skills, ability to problem solve.
  • Current Pharmacist license active and in good standing in the State of practice required
  • Bachelor’s degree or Pharmacy Doctorate from an accredited School of Pharmacy
  • Experience using Framework LTC
  • Experience using Microsoft Word, Excel and Microsoft Outlook
  • Minimum of two (2) years’ recent experience in LTC, PBM, MTM, acute-care setting or one (1) year experience as a clinical pharmacist preferred (not required).

Nice To Haves

  • Management experience preferred
  • Experience and knowledge base of intravenous admixtures services and intravenous therapy is a plus.

Responsibilities

  • Provide clinical expertise by addressing drug-related inquiries from internal staff and external customers, supporting formulary initiatives, and executing utilization management tools like prior authorization programs and drug utilization reviews.
  • Offer clinical consultations via phone, email, or dashboard to prescribers and nursing staff, optimizing medication therapy and deprescribing when appropriate.
  • Verify the completeness and accuracy of new and refill labels, ensuring correct medication packaging and labeling, including emergency drug kits and IV medications.
  • Maintain a resident database with current and past medical history, relevant clinical data, and concurrent medications, including OTC and home remedies.
  • Prepare and/or supervise all sterile IV drug products or solutions and provide evening, weekend, and holiday coverage as needed.
  • Adhere to applicable government regulations, including HIPAA, and perform other duties as assigned based on business needs, ensuring effective workflow and appropriate outcomes.
