Position Summary: TH Case Manager Level 2A serves a dual role at Veterans Place: As a senior case manager, providing direct support and case management services to a designated caseload of veterans in transitional housing programs. As the lead for all housing documentation audits, ensuring that Veterans Place remains fully compliant with the documentation standards of the VA Grant and Per Diem (GPD) program, the VA Contract Bed program, and internal and CARF requirements. This position holds organizational responsibility for internal documentation audits, training support for case managers, and maintenance of service quality across all veteran charts. It also includes group facilitation, housing navigation, community engagement, transportation support, and liaison coordination with the VA under the oversight of the Director of Housing and Support Services. Primary Duties & Responsibilities: Chart Audit Lead & Documentation Oversight As the primary point of accountability for chart audit integrity, this role will: Conduct weekly structured audits of all active and recently discharged charts across all housing programs (GPD, Contract Beds, Community-Based Housing). Use BestNotes to track compliance with required clinical documentation, including: ISP (Individualized Service Plan) creation and updates Weekly/biweekly progress notes Discharge planning notes and summaries Budget documentation Passes and community engagement tracking Medication logs and behavioral notes (if applicable) Create and maintain an Audit Tracker Dashboard identifying trends, overdue entries, incomplete or noncompliant documentation, and missing forms. Report audit findings weekly to the Director of Housing and Support Services with: Corrective action summaries Peer-specific documentation feedback Program-wide trends Provide one-on-one support to case managers to review their charts and resolve deficiencies. Prepare monthly audit summary reports for executive leadership and support CARF/VA audit readiness reviews. Assist with implementing new documentation protocols, updates to BestNotes workflows, and user training. Case Management & Direct Veteran Support Maintain a caseload of transitional housing veterans requiring ongoing case management. Conducted initial needs assessments, created ISPs, and updated progress toward goals monthly. Provide weekly check-ins, documentation of contacts, and collaboration with external providers. Assist veterans in navigating internal and external services, including benefits, housing referrals, mental health services, and reentry supports. Provide support with obtaining documentation (IDs, DD-214, housing applications). Conduct regular room visits and monitor program compliance for assigned residents. Coordinate housing navigation and aftercare services for veterans nearing discharge, including referrals to HUD-VASH, landlord outreach, and housing search support. III. Group Facilitation, Community, and Behavioral Support Lead or co-lead community groups at least once weekly on topics such as: Conflict resolution Budgeting and financial literacy Communication skills Accountability and recovery support Attend and actively participate in: Weekly townhouse meetings Monthly all-program community meetings Address community behavioral issues in collaboration with the Assistant Case Manager Coordinator and Director of Housing and Support Services. Provide veterans with coaching and interventions related to conflict, non-compliance, or behavioral concerns. Coordination with VA and Internal Stakeholders Serve as the primary case management liaison to the VA GPD Liaison for all documentation-related questions or updates related to assigned veterans. Attend bi-weekly treatment team meetings and report on ISP status, documentation compliance, and caseload needs. Coordinate care transitions with the VA, especially for those with complex discharge plans or housing needs. Collaborate with Workforce, VRC, and Facilities teams to ensure veterans have full access to services. Compliance & Budget Oversight for Assigned Veterans Ensure all assigned veterans complete monthly budgeting forms and review with appropriate staff. Track financial behaviors and coordinate financial counseling or support as needed. Maintain complete financial logs and track program service fees, when applicable. Document all veteran-related activities in BestNotes within 24 hours.
