About The Position

Catholic Charities is seeking a self-motivated advocate who is compassionate, trauma-informed, and culturally aware to join our Housing Focused Behavioral Support Team (HBST) at our St. Paul Opportunity Center / Higher Ground - Minneapolis locations. This position is on a PATH Team (under the SAMHSA grant Project for Assistance in Transition from Homelessness) and is also known as the Housing Focused Behavioral Health Support Team. PATH-HBST is onsite Emergency Service for clients of the Catholic Charities Higher Ground Saint Paul Shelter, and the Saint Paul Opportunity Center who live with serious mental illness and often live with additional severe health conditions that require immediate support for the client’s health and safety. This position provides direct case management to adults with serious mental illness and co-occurring conditions who are experiencing homelessness or housing instability. The Case Manager II works alongside clients to assess needs, develop individualized service plans, access resources, and strengthen skills for long-term stability. They serve as both advocate and connector - helping clients navigate complex systems of care while promoting dignity, safety, and independence. The role collaborates closely with internal teams and community partners to ensure clients receive consistent, trauma-informed support. This position requires a valid Minnesota driver’s license, a reliable vehicle, and proof of insurance.

Requirements

  • Bachelor’s degree in human services or related field preferred, with a minimum of two (2) years of experience working with a specific client group.
  • In lieu of degree, three (3) years of directly related experience required.
  • Must have a valid driver’s license and proof of current insurance if transporting clients.
  • Experience working with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, and/or mental illness.
  • Additional training, education, or licensure may be required depending on program needs.

Responsibilities

  • Completes assessments, prepares, evaluates, and maintains individualized service plans that meet client needs using both Catholic Charities and community resources.
  • Supports and advocates on behalf of clients to obtain services, benefits, and supports — assisting with documentation and applications as needed.
  • Coordinates referrals and partnerships with community providers to ensure comprehensive care.
  • Maintains accurate case documentation and records, meeting all agency, funder, and licensure requirements.
  • Provides education and resource information to clients, volunteers, and community partners.
  • Collaborates closely with supervisors and team members for consultation, coordination, and case planning.
  • May transport clients to appointments or community resources when appropriate.
  • Engages community systems for linkage, referral, crisis management, and follow-up.
  • Provides assessment, education, and case management services to a complex, often long-term caseload, which may include in-home visits.
  • Advocates for clients to promote stability, safety, and equitable access to resources.
  • Performs other duties as assigned.
