This is a work-from-home role with community travel to meet members in their homes and to DCS locations as needed. What You’ll Do Provide peer support and mentorship grounded in your lived recovery experience Assess needs, complete screenings, and help develop care plans Connect members to community resources and essential services Collaborate with care teams, primary care providers, managed care organizations, and community partners to ensure coordinated, high-quality support
