Telehealth Physician (Full-Time | M-F | No On-Call)

Requirements
- MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Active medical license — multi-state licensure a plus
- Credentialed with major plans: UHC, Aetna, Humana
- Strong interest in preventive care, senior health, or chronic condition management
- Tech-savvy and comfortable practicing via telehealth
- Collaborative, mission-driven mindset with excellent communication skills
Nice To Haves
- Active medical license — multi-state licensure a plus
Responsibilities
- Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
- Create and manage personalized care plans for ongoing conditions
- Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
- Ensure thorough documentation in our EHR and compliance with quality measures
- Engage patients in preventive care to reduce avoidable complications
